Safeguarding

The Safeguarding of Children and of Vulnerable Adults is extremely important to us.

If you wish to discuss any concerns around the safety of Children or Adults please raise them with your GP. They will be able to advise you on how best to proceed.

If you are a young person or an adult who feels threatened or at risk in your home or any other environment please feel able to discuss this with your GP as soon as possible.

Medical Records

Confidentiality

It is one of our highest priorities that you should feel that your information is secure; therefore Lantern Health complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:

    • To provide further medical treatment for you e.g. from district nurses and hospital services
    • To help you get other services e.g. from the social service. This requires your consent.
    • When we have a duty to others e.g. in child protection cases.
    • Anonymised patient information will also be used at local and national level to help the Health Authority and NHS plan services e.g. for diabetes care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Access Medical Records

All Lantern Health staff have access to your medical and personal details which is required in relation to their roles, and have completed confidentiality agreements.

Information will not normally be released to other family members without written patient consent.

Lantern Health has a policy of openness with regard to health records and health professionals are encouraged to allow patients to access their health records on an informal basis.

To access copies of your Medical Record you must do this in writing to the Operations Manager

Summary Care Record

Your SCR contains important information about your health, any medicines you are taking or any allergies you suffer from. Consent to share your SCR allows authorised healthcare staff to have access to this information and will improve decision making by doctors and prevent mistakes being made when patients are being cared for in an emergency or when their GP practice is closed.

You can choose to have a SCR or you can choose to opt out. If you choose to have a SCR and are registered with a GP practice, you do not need to do anything as a SCR is created for you. If you choose to opt out of having a SCR you need to let your GP practice know filling in and returning an opt out form. If you are unsure if you have already opted out, you should talk to a member of our Lantern Health Clinical Team. You can change your mind about having or not having a SCR at any time by formal notification of your wishes to our Operations Manager.

All patients at Lantern Health Practices have a named GP who is responsible for their overall care at the practice – please contact the practice if you would like to know who this is. If you have a preference about your named GP please speak to Reception and the practice will make reasonable efforts to accommodate your request. As a patient of Lantern Health you will be treated with courtesy and respect. We will involve you and listen to your opinions and views in all aspects of your medical care.

Our team at Lantern Health will advise and inform you of the steps you can take to promote good health and a healthy lifestyle.

Your Commitment to US

We would respectfully ask that you:

  • Let us know if you intend to cancel an appointment or are running late.
  • Inform Lantern Health staff of any alterations in your circumstances, such as change of surname, address or telephone number. Please ensure that we have your correct telephone number.
  • Treat staff with courtesy and respect.

We are confident that you will treat Lantern Health staff with the same courtesy and respect we show you. Although this is extremely rare, if our staff are subjected to any form of abuse of violent behaviour we reserve the right to adhere to the NHS Zero Tolerance Policy.

NHS Zero tolerance policy

The NHS operates a zero tolerance policy with regard to violence and abuse. Lantern Health therefore reserves the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.
